Procedure for Disclosure of Personal Information

1. Request for Disclosure

To make a request for disclosure, please send by mail only a prescribed form with the required documents enclosed to the address below. It would be helpful if such superscription as “Request for Disclosure of Personal Information Enclosed” is written in red on an envelope.

2. Documents (including a Form) to be Submitted for the Procedure for Disclosure

When making a request for disclosure, please download the form (1) specified below, fill in the required fields, and send it to the address shown in the Paragraph 1 above by post together with the document (2) required for identity verification enclosed.

(1) Prescribed disclosure request form

If you are unable to download the form, please contact the Personal Information Office of Mitsui Fudosan Realty Group of Companies.

• The Disclosure Request Form must be filled out without any omission and signed and sealed by the applicant.

(2) Identity verification documents

• A photocopy of a driver’s license, passport (on which an address is described), or health insurance certificate (the code and number of the insured must be blackened out in advance; the same applies hereinafter): One copy

3. Request for Disclosure by Proxy

If a person making a request is a statutory agent of a minor or adult ward, or an agent delegated by an applicant with regard to the request for disclosure, please enclose the documents as specified below (either (1) or (2)), in addition to the documents listed in the Paragraph 2 above.

(1) In case of a statutory agent

• A document to prove the authority of statutory representation (a certified copy of family register; or in case of a person with parental authority, a photocopy of the health insurance certificate, on which the names of the dependents are specified, is acceptable): One copy
• A document to identify the statutory agent (a photocopy of a driver’s license or health insurance certificate of the statutory agent): One copy

(2) In case of an agent based on delegation

• Letter of proxy: One copy
• A document to identify the agent (a photocopy of a driver’s license or health insurance certificate of the agent): One copy

4. Fees and Payment Method

• Any postage for sending documents to the Company and issuing commission for a money order shall be borne by an applicant.
• When the amount of the fees received is insufficient or no fee is enclosed in the request documents, the Company will notify such applicant in question. If fees are not paid in full within the prescribed period of time, the request shall be treated null and void.

5. Method of Response

The Company will respond to the request via postal mail to the return address designated in the Request Form for Disclosure of Personal Information.

6. Purpose of Use of Personal Information Acquired during the Course of a Request for Disclosure

Any personal information the Company obtains during the course of a request for disclosure shall be utilized only to the extent necessary for the request for disclosure.

7. Nondisclosure of Retained Personal Data

Any information will not be disclosed in the following cases. However, the prescribed fees shall be charged in such cases as well.

• When the address on the request form does not correspond to the address shown in the document for identity verification so that the identity of the applicant cannot be verified
• When power of attorney is not confirmed in the case of application by an agent
• When the subject of the request of disclosure does not constitutes the Retained Personal Data
• When it is feared that damage is inflicted on the life, body, property, or other rights and interests of the applicant or a third party
• When it is feared that the normal course of the Company’s business is materially hindered
• When disclosure would constitute a violation of any other laws and regulations
